Middle School Athletics Registration & Schedule Information 

Medical Clearance

Only the PPE Medical Eligibility Form and/or the Health History Update must be submitted to the RRMS school nurse.

• PPE- Medical Eligibility Form
• Health History Update Form
• Preparticipation Physical Evaluation Form     
• All physical exams performed on 7/1/2024 or later must be documented on this form as per NJDOE.
• A copy of the Preparticipation Physical Evaluation form is not required to be submitted to the nurse but is highly recommended.

As a reminder, all physical examinations are valid for 1 year from the date of the exam.  If the exam date is older than 90 days from the first official date of practice, a Health History Update form will be required.  Students will NOT be permitted to try out or participate in interscholastic athletics without completing both steps of the process outlined above and being cleared by the school nurse.

• Completed medical forms can be:

  • Dropped at the main office during summer hours.

  • Mailed USPS to Ramapo Ridge Middle School, Att: K Jaffe – 150 Ridge Road-Mahwah NJ 07430.

  • Emailed as an attachment to rrathletics@mahwah.k12.nj.us

  • Please do NOT email pictures of Physical Exam forms
